Department of Medicine/Division of Cardiology is seeking an experienced nurse practitioner or physician assistant to work in our cardiac disease management programs. (3-5 years cardiology experience required). This is a full-time position (no weekends, no holidays and no call) under the supervision of the Medical Director of Cardiology Services. Candidates with experience in cardiovascular medicine with an ability to work independently are highly desirable and are immediately needed to support patient care. This individual will play an essential role in managing a referral based practice for patients requiring cardiac and vascular care. Competitive compensation and benefits package offered. The provider will assist in the development and coordination of Disease Management Programs within the Heart & Vascular Institute and will collaborate with members of a multidisciplinary treatment team to provide exceptional treatment, education and support to patients with complex, acute, critical and/or chronic health conditions. The provider must demonstrate a high level of clinical proficiency, and excellent decision-making skills.  Clinics will include direct care and treatment coordination for chronic heart and vascular conditions including: •Congestive heart failure •Peripheral vascular disease •Heart valve disease •Hypertension/Pulmonary hypertension •Heart disease in women •Hyperlipidemia •Dyspnea ioner or physician assistant to work in our cardiology department at AOMC.  Highly competitive compensation and benefits package offered.

Completion of Masters Degree from accredited program

Current Nurse Practitioner or Physician Assistant certification required

Active NYS license
